• To monitor and ensure communication and collaboration between clinicals, referring professionals and clinical outreach team regarding patient aftercare planning.
• Provide administrative and communication support for onsite referral relation team and outreach representative.
• Begin to establish patients needs and goals for aftercare upon admission.
• Coordinate and monitor aftercare including follow-up appointments with providers and recommendations.
• Liaison for patients and their families to various community agencies, hospital consultants and services.
• Develop a comprehensive discharge plan including where the patients is discharge to, how they will get there and who they will see in the next phase of the patient's treatment.
• Demonstrate ability to develop a comprehensive discharge plan to be submitted to treatment providers upon discharge.
• Conduct follow-up calls with clients/patients and build a referral base with the other organizations.

Responsibilities:

• Communicate with UR Team to ensure getting a right information in regards to patients authorizations.
• Trained a new Care Coordinator.
• Communicate with Strategic Relations Manager to coordinate aftercare planning.
• Communicate with patients, therapist, onsite referral relation director, referring professional and clinical outreach to ensure and provide appropriate care and appropriate referral options upon discharge.
• Complete regular internal KIPU audits.
• Manage patient assignment between Residential and Outpatient to ensure new patient assign to Discharge Planners.
• Managing Veterans, Active-duty Military ,EAP (Pilot/Airlines Employee and Chrysler Employee) to ensure referent and referring professional are communicated regarding patient aftercare planning. Airlines: To ensure patient provided with appropriate referral to a facility that is experience with Airlines (EAP/FAA/HIMS). Veterans: Collaborate and Communicate with the assigned therapist and the VA to confirm which VA Program patient will continue. Chrysler: To ensure referent and referring professional are communicated regarding patient aftercare planning with appropriate referral experience working with EAP.

• Report absences/refusal to PCC and Clinical Director.

• Assist with MMPI testing for all new patients.
• Assist Family Program contacts and executions.
• Assist with Aftercare Pilot program and reporting.

• Responsible for overall Rose Garden community.
• Responsible for Compliance Ensures community operation and program achieved compliance expectations as measured by State and Federal.
