Provide screening and assessments for students having difficulty with social and emotional functioning in school or home environment.
Collaborate and developing a recovery-based treatment plan of care with student centered goals and objectives.
Consult and collaborate with parents, teachers, administrators, community leaders and other stake holders to promote and support student success.
Provides medically necessary clinical services including but not limited to assessment, individual and family therapy.
Perform therapeutic interventions that are directly related to the students social and emotional distress.

Provide triage, crisis intervention, suicide, self-injury assessment and therapy services as part of Fort Mill School District protocol.
Provide medically necessary clinical services, including but not limited to, assessment, individual, family and group therapy and crisis intervention services to a targeted population. Services may be provided in the clinic, in the client’s home, or other community setting. Responsible for case management services for assigned caseload. These services include: referrals, advocacy, interagency staffings, and coordinating care between agencies/service providers as appropriate. Formulate recovery-based plans of care with person centered goals and objectives. Conduct timely reviews and maintain accurate clinical documentation. Attend and participate in Treatment Team meetings / staffings, including staffing with MD at CCMHC. Pursue continuing education and training. Maintain positive professional relationships with MHC staff, and community agencies, as well as, clients and their families. Work to overcome the stigma of mental illness and mental health services by providing education, training and support services to the community at large.
Provide triage, crisis intervention, assessment and therapy services as part of York Adult intake/triage team.
Provides emergency assessments of clients experiencing crisis. Conducts face to face consultations as needed, arriving on-site no later than 60 minutes after the receipt of call. Formulate level of care and disposition for Mobile Crisis calls. Educate patients on community resources and make arrangements for immediate disposition regarding transfers and referrals. Serve as provider of crisis intervention by administering brief individual and or/family therapy to address crisis; identify triggers for crisis behavior to include development of crisis safety plan up to initiating the process of involuntary commitment as needed. Maintain call log for quarterly crisis report.
Provide follow up calls to individuals served by Mobile Crisis. Engage in outreach contacts with community partners/agencies to increase awareness of Mobile Crisis services available in Chester, Lancaster, and York counties. Provide these services as needed during monthly visits to Chester and Lancaster clinics.
Attend and participate in weekly clinical treatment team meetings and staff patients with CCMHC MD as needed. Pursue continuing education and training. Maintain positive professional relationships with CMHC staff and community agencies as well as patients and their families. Work to overcome the stigma of mental illness and mental health services by providing education, training and support services to the community at large.
Conduct screenings, Investigations and assess reported adult abuse/neglect or exploitation cases. Determine the validity of such reports and investigate circumstances surrounding alleged adult neglect and/or abuse; complete agency reports as required; develop, implement and update individual service plans and monitor progress toward treatment goals;
remove adults from abusive or neglectful homes in order to place them in a protective environment; and maintain case records, case narrative recordings, written and electronically,
Input them into the agency designated system for documentation and auditing purposes as required by agency policy in a timely and accurate manner. Provide crisis management for clients and linkages for intervention as appropriate with service providers. In addition to, assisting law enforcement and/or Family Court in emergency removal of adults and placement in a protective environment.
Provide individual therapy to clients who are on probation, has a felony and/or chronic misdemeanor offenders. Formulate a multidimensional impression surrounding a history of mental illness, medical treatment, physical/substance abuse, suicidal attempts/ideation and more. Assists patients in coping with stress related to hospitalization following a crisis. Provides educational and other information to better assist patients in defining goals and implementing action plans to achieve personal, social, educational and/or vocational development and adjustment.
Participates in multidisciplinary care planning meetings. Completes referrals and authorizations as needed.
