• Provide direct nursing care to assigned patients in multi-specialty outpatient clinics such as: Orthopedic, Podiatry, Otolaryngology, Oral and Maxillofacial Surgery, Plastic Surgery, Rheumatology, Cardiology, Dermatology, Neurology, Vascular Surgery, Endocrinology, Colorectal Surgery, Ophthalmology, Nephrology, Radiation Oncology, Urology, Hematology, Infectious Disease.
• Exhibit strong clinical judgment by accurately triaging patients and escalating concerns to higher levels of care when necessary.
• Participate in assessing, planning, implementing and evaluating the nursing care given.
• Perform admission, discharge, and transfer procedures.
• Utilize nursing plans of care in daily assignments, able to review, and, under supervision, modify the plan of care.
• Observe and record the signs, symptoms, and behaviors including
the physiological status of patients, present proposed revision of interventions, and desired outcomes.
• Deliver the designated nursing interventions to assigned patients that are consistent with the stated medical plan of care.
• Notify appropriate nursing and medical staff of changes in patient status; recognizes patient care priorities, multidisciplinary treatment plans, and potential patient care crises.
• Maintain patient's privacy and confidentiality of information and records at all times.
• Safely and properly administer medications and treatments.
• Execute written physician's orders for assigned patients.
• Assemble equipment and supplies for various diagnostic or treatment procedures in the specialty service area; perform/assist others with procedures/treatments.
• Adhere to required infection control, isolation procedures, and aseptic techniques in the delivery of patient care.
• Adhere to internal controls established for department.
• Mentor medical students and residents, fostering their professional growth and development as future healthcare providers.
• Demonstrate adaptability and resilience during periods of high patient volume, effectively managing multiple tasks simultaneously while maintaining a focus on delivering quality care to each individual served.
Engage, enroll, and maintain members in active participation in the OnTrak program as well as to coordinate member care.
Utilizes motivational interviewing, enhancement, and relapse prevention guidelines in telephonic communication with clients.
Usage of Catasys OnTrak treatment program and usage of the eOnTrak electronic documentation platform.
Provide exemplary customer service to and member-centric focus for, all clients.
Communicates or requests information from providers in complete compliance with confidentiality standards and in a manner that generates cooperation
Works jointly with the Clinical Operations Coordinator and Community Care Coordinator to support member referral, engagement, and enrollment into the OnTrak program.
Supports enhancement of the digital tools eOnTrak by providing feedback to the development team and performing ad hoc system testing as indicated.
