• Complete a comprehensive assessment on the HIV client’s initial visit, including: history and physical exams, HIV risk behaviors, mental health and substance abuse needs, baseline laboratory evaluations, referrals to dental clinic, nutritionist, case manager, and vaccinations
• Collaborated with the HIV Case Manager to assess the individual needs of every HIV client and in turn help implement a plan of care accordingly, considering all barriers to the achievement of health and quality of life
• Collaborated with the HIV Case Manager to act as an advocate for the HIV clients in all aspects of their health and life
• Responsible for the completion of the H.R.S.A
• Annual Report “Ryan White Data Report” for the past four years for Title C Services
• An active participant in writing the Work Plan for our 5 year Title C Grant.
