• Ensures members are receiving services in the least restrictive setting in order to achieve and/or maintain optimal well-being
• Planning and implementing interventions to meet those needs
• Coordinating services, and monitoring and evaluating the case management plan against the member's personal goals
• Guides members/families towards resources appropriate for their care
• Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams and others involved in the member's care as appropriate and required by our comprehensive contract

• Assisted families in crisis situations by providing immediate support and coordinating with relevant community resources.
• Complete Home studies- will complete 45-day home studies & placement home studies.
• Relative searches- will need to be completed every 90 days for children in foster care &/or recruitment
• Documenting all efforts in FSFN for searches completed and responses had from the letters
• Assist with seeing incarcerated parents and entering notes in FSFN
• Assist with weekly efforts of missing children, documenting efforts in FSFN, and completing Risk Assessments once children are found
• Assist with IL tasks needed for children aging out of foster care, assist with setting up the meetings with FSS, and obtaining all documentation that the child will need prior to the child aging out of care
• Ensuring that data measures are entered in FSFN and assist with getting it uploaded. This would include: educational records, medical records, dental records, etc.

• Remained up-to-date on changes in relevant legislation impacting the field of child protective services
• Served as a mentor for new investigators by providing guidance on proper protocols for handling cases
• Conducted home visits to evaluate family dynamics; monitored compliance with court orders or voluntary service agreements

• Monitored activities to maintain safe living and learning environment for youth
• Served as role model by demonstrating appropriate examples for youth
• Monitored daily activities of participants and documented any changes or issues that arose
• Developed positive relationships with youth by engaging in recreational activities
