Assisted in opening the new MAT Access Point also known as SUD Wellness and Recovery Access point and continuing to provide MAT services / inductions to adults and youth in north, west, and south county of Santa Barbara. Planned, organized, implemented and documented all MAT appointments. Documented observations of clients within our MAT Access Point in medical charts by utilizing the electronic health record system. Set clients up with an appointment Via telehealth with a prescribing physician, perform weekly check-ins with clients and family to provide support and education regarding MAT. Assist clients in completing online forms and applications. Support with obtaining releases of information in order to assist with appropriate linkage to community-based organizations. Support with completing and obtaining the Informed Consent for treatment with clients in order to provide the best support to the individual. Provide recovery services to clients including but not limited to; assistance with community linkage, transportation arrangement, appointment reminders, encouragement to utilize coping skills as well as support services and engagement in treatment. Engage in treatment planning and organizational meetings via Zoom and in person in order to discuss client care and system management support. Utilization and creation of evidence base curriculum, Participated in community organizations and prevention strategies, Participated in the creation of YOR Place youth opioid treatment program and continue to provide services to youth and families, conducted community outreach and engagement regarding the services the program provides, conducted educational presentations to youth, families and community members, conducted individual and group treatment sessions to enrolled youth. Conducted groups out on school sites during drop-in hours. Assisted and managed monthly Narcan distribution events, educated community members on the use of Narcan. Perform screenings, assessments and treatment services to youth and young adults and determined the needs of care and provided the necessary services at YOR place or provided the necessary referral to appropriate level of care. Maintained positive relationships to community organizations such as local schools, probation departments, juvenile Halls, mental health organizations, SUD treatment providers, to create low barrier access to youth, young adults and families seeking treatment or support. Provide support in Care Coordination services to support Medi-Cal beneficiaries with navigating the Alcohol and Drug program system of care and getting connected to programs that meet their needs such as linkage to primary care, perinatal care, mental health treatment, social services, shelter, food services and transition into higher levels of care when needed. Complete ASAM screenings and Bquips (assessment screening) for clients to identify and appropriate level of care, complete Medi-Cal verification for clients to ensure they meet need for services and provide resources in the community to individuals who do not have Medi-Cal. Review and editing of program policies and procedures to ensure that all programs are in compliance.
