Assisted in opening the new MAT Access Point also known as SUD Wellness and Recovery Access point and continuing to provide MAT services / inductions to adults and youth in north, west, and south county of Santa Barbara. Planned, organized, implemented and documented all MAT appointments. Documented observations of clients within our MAT Access Point in medical charts by utilizing the electronic health record system. Set clients up with an appointment Via telehealth with a prescribing physician, perform weekly check-ins with clients and family to provide support and education regarding MAT. Assist clients in completing online forms and applications. Support with obtaining releases of information in order to assist with appropriate linkage to community-based organizations. Support with completing and obtaining the Informed Consent for treatment with clients in order to provide the best support to the individual. Provide recovery services to clients including but not limited to; assistance with community linkage, transportation arrangement, appointment reminders, encouragement to utilize coping skills as well as support services and engagement in treatment. Engage in treatment planning and organizational meetings via Zoom and in person in order to discuss client care and system management support. Utilization and creation of evidence base curriculum, Participated in community organizations and prevention strategies, Participated in the creation of YOR Place youth opioid treatment program and continue to provide services to youth and families, conducted community outreach and engagement regarding the services the program provides, conducted educational presentations to youth, families and community members, conducted individual and group treatment sessions to enrolled youth. Conducted groups out on school sites during drop-in hours. Assisted and managed monthly Narcan distribution events, educated community members on the use of Narcan. Perform screenings, assessments and treatment services to youth and young adults and determined the needs of care and provided the necessary services at YOR place or provided the necessary referral to appropriate level of care. Maintained positive relationships to community organizations such as local schools, probation departments, juvenile Halls, mental health organizations, SUD treatment providers, to create low barrier access to youth, young adults and families seeking treatment or support. Provide support in Care Coordination services to support Medi-Cal beneficiaries with navigating the Alcohol and Drug program system of care and getting connected to programs that meet their needs such as linkage to primary care, perinatal care, mental health treatment, social services, shelter, food services and transition into higher levels of care when needed. Complete ASAM screenings and Bquips (assessment screening) for clients to identify and appropriate level of care, complete Medi-Cal verification for clients to ensure they meet need for services and provide resources in the community to individuals who do not have Medi-Cal. Review and editing of program policies and procedures to ensure that all programs are in compliance.

Perform variety of duties to support Drug and Alcohol clients and staff including but not limited to; Outreach work; visiting substance users in the community/at the clinic; helping with immediate needs; drop in center work, which includes talking to clients about their needs and finding ways of supporting them towards recovery as well as accessing community resources; Counseling and rehabilitation through giving support to clients dealing with the causes of substance misuse; Education and training; Helping clients access needed services; Case management; Basic principles and practices of customer service; Identifying and assessing social needs; Assisting with linkage to community resources; Fundamental recordkeeping and documentation in line with medical billing; HIPPA compliance and 42 CFR; Organize and prioritize work assignments to meet deadlines as well as work independently; Establish and maintain effective working relationships with a wide variety of professionals, clients, colleagues and representatives of various agencies and/or departments; Assist individuals with a wide variety of needs including but not limited to substance use, mental health, medical issues, legal issues, developmental disabilities; Work effectively with individuals from various socio-economic and/or cultural backgrounds; Access and use basic personal computer applications including word processing; Inputting and retrieve data and information stored in computerized record systems.

While working in this locked facility I was responsible for; Monitoring and administering medications for the assisted living and memory care facility; Kept accurate records of new medications, inventory of all medical supplies needed, clients current functioning levels, medical issues, records, appointment's and histories; Conducted medical intakes and new patient intakes which included complete medical histories and assessed/screened for substance use disorders; Completed monthly medications cycles; Re-ordered all the residents medications and prescriptions; Evaluated clients for medication and substance use misuse or abuse; Provided psychoeducation on the interactions of medications and the importance of adherence to medications; Implemented appropriate interventions to address substance use issues or misuse of medications; Input new residents medication list into our ALMSA Emar system; Provided customer service/tours to potential new patients and their families; Set up appointments for woundcare, physical therapy, and doctor appointments; Responsible for making sure the care staff get all their daily assignments done in a timely manner; Support co-workers and colleagues to engage in team based care for patients; Provided and engaged in accurate and complete documentation of all services provided; Used time management skills to support patients in receiving quality care, as well as to prioritize work assignments to meet work assignments; Communicate and work with supervisors, doctors, nurses, community organizations and co-workers to ensure that the patients’ needs are being met and protocol is being followed, as well as care plans are being implemented appropriately; Responsible for monitoring hospice and dementia patients for change of condition; Responsible for office duties such as faxing, emailing, scanning, answering phones and providing customer service to patients, families, community organizations and partnerships; Read and understand patient records to build the proper patient centered treatment plan; Ensure that the clients plans were being followed as each client’s directives were different (some were conserved) others had DNR, ect. Served as liaison between client and community based organizations.
