• Engage in multidisciplinary collaboration with Stroke and Cerebrovascular Center (SACC) team to help facilitate the integration of psychosocial supports within the care model
• Complete clinical evaluation of SACC patients and families to assess for a variety of concerns, including mental health, child protection, intimate partner violence, and acute resource needs
