• Monitor and document patients vital signs.
• Administer medication and provide treatment as prescribed.

• Assess patients conditions and provide accurate documentation of patients health.

• Perform wound dressing change and monitor infections and treatment programs.

• Develop and revise care plans.

• Participate in an interdisciplinary team of physicians, therapists, dietician, social workers among others to improve patient outcomes.

• Monitor and provide direction to nursing staff to ensure patient cares are completed.

• Assess and monitor patient conditions and communicate effectively to physicians any change in conditions.

• Educate patient and families on the treatment program for a successful discharge.

• Administer medication and treatments per doctor’s orders.

• Perform dressing changes and treatments as ordered.

• Monitor and Document patient vital signs and their conditions and communicate any changes to physicians.

• Communicate with patients, families and interdisciplinary team to improve patient outcomes.
