• Coordinate RTW for Employee who are out on personal medical leave
• Other duties as needed in a busy warehouse of over 450 employees.
• Coordinated medical evaluations and follow-up care for injured employees.

• Assist Physicians and Nurse Practitioner's with on-site injuries.
• Review pre-employment physicals, contact applicants for missing vaccines and update records
• Administer TB testing and vaccines for new hire employees.

• Prioritize patient concerns, redirect as appropriate and gain insight through comprehensive and direct questions
• Particularly well versed in provision of telephone care to the elderly
• Skilled in Microsoft Office, typing, excellent phone skills, emergency care, and patient centered care
• Phlebotomy, Front and Back office skills, Medication refills.

• Manage Random Drug and Alcohol Program for Offshore employees
• Manage and complete audits when necessary for NCMS, DISA, CREW, OHM, ASAP for Offshore Drug and Alcohol Compliance.

• Provide support to Physicians, Medical Office Staff and Care Coordinators
• Gathering Information and participating in outreach to patients/families, facilities and other members of the health care team
• Ensures quality ambulatory care coordination to populations managed and targeted within programs at One Memorial, adhering to policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice
• Promote individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff
• Participates in discharge planning activities (e.g
• Contact of post-acute care facilities, other community resources and transportation) as needed to ensure safe patient transitions from hospital to home and appropriate linkage with post-acute providers
• Provides support to CM staff by faxing, copying charts, filing, scanning and follow up on individualized plans of care
• Participates in performance improvement activities of the department and institution.
