Report changes in the elder’s condition to the clinical support team per neighborhood change in condition protocols.


Assist elder with personal care functions, including dental and mouth care, bathing, dressing and grooming, bowel and bladder care and other personal and hygiene care.


Meet with the clinical support team as scheduled or needed, to discuss elders needs and progress in identifying and correcting problems attend and contribute to the elders’ scheduled care plan meetings.


Report accidents and incidents when they occur.


Assist with and/or perform restorative and rehabilitative procedures as outlined by the service agreement.

• Fulfill customer orders by reviewing orders, finding, preparing, and packaging merchandise.
• Ensuring the accuracy of orders before pick up, making proper product substitutions, and consulting with customers as needed to ensure satisfaction
• Placed completed orders in labeled, temperature-appropriate storage pending customer pick up.

• Providing residents with routine daily nursing care per the resident's assessment and plan of care, and directed by the department director or supervisor.

• Implements identified plan through coordination of care with interdisciplinary care team to employ strategies to promote health and wellness.

• Communicate pertinent data and information related to the patient, situation, or setting promptly.
• Provide assistance with activities of daily living (ADL’S), accurate measurement and recording of weight, height, vital signs and report any changes to assigned nurse
