Report changes in the elder’s condition to the clinical support team per neighborhood change in condition protocols.
Assist elder with personal care functions, including dental and mouth care, bathing, dressing and grooming, bowel and bladder care and other personal and hygiene care.
Meet with the clinical support team as scheduled or needed, to discuss elders needs and progress in identifying and correcting problems attend and contribute to the elders’ scheduled care plan meetings.
Report accidents and incidents when they occur.
Assist with and/or perform restorative and rehabilitative procedures as outlined by the service agreement.
