I am currently a registered nurse working for the Portland VA healthcare system at the Purchased geriatrics and extended care (PGEC) department in Vancouver, WA as a nurse care coordinator. Having worked around people in various positions, I am used to dealing with individuals and groups of people. I am able to use my experience with patients and their families to keep them calm and as comfortable as possible. I can stay composed and focused in high stress situations. I remain compassionate and understanding even when dealing with difficult patients and situations. - Purchased non-institutional care (PNIC) I coordinate non-skilled home and respite care, Veteran-directed care, community adult day health care and the caregiver support program for the initial start and reauthorization of care. I receive and process requests for additional caregiver hours utilizing the case mix tool to determine hours for in home caregiving. I act as a customer service liaison to mediate problems between private caregiving agencies and patients.
- Community health care (CHC) I coordinate skilled home care for veterans to include, skilled nursing, physical therapy, occupational therapy, Social work, Home health aide, Infusion and hospice care for initial start and reauthorization of care and processing requests for additional services and mediate problems between agencies and our Veteran customers.
- Community nursing home (CNH) I coordinate placement and assessment/reassessment of veterans placed in nursing homes by the VA and conduct site visits to evaluate and assure the quality of care for veterans placed in nursing homes in the community. I act as a liaison for nursing home staff to facilitate requests for service. I initiate and renew contracts for payment from the VA to the private sector using Veteran care agreements, Triwest and IDIQ contracts.
