  • As a Transition Coordinator, we meet the unique needs of delinquent youth transitioning from residential programs back into their community. For a Lead Transition Specialist, we supervise the Transition Specialists to make sure they are meeting the needs of all youth returning to the community.

  • Performs functions involved with assessment, including initial contact with the youth, facility personnel, and parent/legal guardian regarding the youth’s needs, presenting problems, and any other related issues when referral is received from the facility. Referrals are received at least 90 days prior to discharge. Provides supervision for juvenile offenders release from Department of Juvenile Justice Program that are placed on conditional release, post commitment probation, or probation. Formulates intervention plans and conducts assessments. Lead Case Manager handles the more complex and difficult case problems, provide technical guidance and training to lower classes, and may act as a shift leader. Lead Case Manager differs from the next higher class, Supervising Case Managers, in that the latter is the first line supervisor responsible for the coordination, direction, scheduling and supervision of either field services, a special programs, or an institution.
