Check and record patient’s vital signs such as blood pressure, temperature, oxygen, respiration, weight and report to nurse.

Chart patient vitals and activities for the shift and report improvements and any changes to nurse

Assist patients with personal hygiene like bathing, brushing teeth and dressing.

Assist patients with meals by feeding according to dietary needs

Reposition, move and turn patient from bed to seat with the help of nurse

Check patient blood sugar and report findings to nurse

Ensure 2 hourly round check on patients to and provide help to patient

Follow patient strict output of fluids communicated by the nurse

Assist client with daily personal grooming such as bathing, brushing teeth and dressing

Perform household task such as laundry, washing dishes and keeping a clean environment

Shop for groceries according to client’s dietary needs

Help to organize client’s schedule and appointments to doctors office

Report changes in client’s condition to supervisor

Help with simple exercises

Administer clients medication under the supervision of the supervisor and case manager
