  • Opened Denovo unit with home therapies in 2016
  • As of August 2024, the unit has grown to 35 patients
  • Increased utilization of connected health, reduced hospital readmissions and increased home therapy retention
  • Managed patient caseloads effectively, ensuring timely interventions and thorough follow-ups to mitigate potential complications.
  • Traveled to area clinics to train new staff and patients in order for them to get CMS certified
  • Being the only RN from 2016-2020, prepared data from Equip for Clinical Quality Improvement meetings, gave Governing Body Home Therapy information to Facility Administrator and working with clinical quality leadership team
  • Assisted at other HT units needing assistance, continue to train both PD and HHD patients, case manage patients while keeping outcomes good
