  • If there are new ulcers, check if resident is on turning/positioning program (select the intervention with a pencil), appropriate mattress and appropriate treatment

  • If there are new admissions the day before, or the Friday before, check the completed Braden Scale and ensure a skin assessment was accurately documented and review the care plan interventions related to skin integrity
  • It is especially important to determine if the resident’s skin injury or pressure ulcer is hospital-acquired or facility-acquired
  • Pressure ulcer/injury impacts the facility legally, financially and the facility’s compliance to the regulations
  • The number of resources used to treat pressure ulcers/skin injuries cannot be underestimated
  • Therefore, all new admissions/readmissions should be evaluated for presence of skin injury
  • If the Wound Care Coordinator finds no skin issues, a progress note will be written documenting the assessment the same day
  • WEEKLY: Prepare and print wound care tracker for rounds
  • In addition, you will go into the E-TAR and compare the treatment on your record to the one that is written in the E-TAR or TAR
  • If there is a discrepancy, correct it immediately and E-mail the RN Supervisor and Charge Nurse responsible for the unit and copy to the DNS

  • Scheduled to collaborate with you the day of Wound Rounds to help you with positioning the resident
  • MONTHLY: Run the Braden Scale report to ensure all residents were accurately assessed
