  • Responsible for directing clinical activities of parent agency and branch office, including but not limited to planning and evaluating professional health services and conferring with leadership and other staff to ensure services are provided at quality levels consistent with professional standards and goals.
  • Provide clinical oversight and support for nursing, social work, spiritual care, bereavement, and volunteer services as well as care transition coordinator roles (nursing and non-nursing).
  • Provide oversight for the hospice program regarding admission, death, and care processes including facilitation/participation in interdisciplinary group meetings. Conduct on-site staff and patient clinical assessment and reassessments as needed. Communicate with physicians and mid-level providers, monitor direct patient care, creates and/or implement staff education, help deliver cost-effective, high quality care to patients and families.

  • Provide support for palliative program, including review of hospice-eligible patients through the interdisciplinary group process and develop strategy to transition those patients to the appropriate level of care.
  • Coordinate overlapping needs for hospice, palliative, and home care services.
  • Supervise leadership /coordinator/team leads and the programs they oversee, provide assistance with development, implementation, and evaluation of supportive processes for all team members. Provide coaching/staff development opportunities.
  • Develop/oversee/guide direct care staff performance development in conjunction with Patient Care Managers, including participation in counseling and disciplinary actions as needed.
  • Monitor compliance with rules and regulations of federal and state regulatory agencies and accreditation bodies. Assure completeness and accuracy of the medical record. Co-responsibility for quality assurance (QAPI) program.
  • Work to define quality metrics and implement programs to address quality indicators set by CMS.   • Investigate clinical concerns and events, including conducting interviews and creating chronology of events. Report findings and recommendations to divisional leadership as needed.
  • Participate in growth activities to ensure profitability of agency including community outreach and education and sales/marketing strategy development.
