Assist lead teacher in coordinating and implementing educational curriculum. • Documenting, auditing and maintaining extensive written records/reports regarding service coordination and all other Early Intervention (EI) activities; Implementing and monitoring all child and family EI services within established timeframes; Performing monthly/regularly contacts with all families, providers, EIODs, school district personnel, etc. as needed; Providing families with regular social/emotional/financial/educational resources as appropriate for their needs; Leading biannual reviews and other team meetings as needed; Conducting initial and regular home visits with families as needed; Facilitating transition from the EIP to Preschool Special Education services prior to the child's third birthday

• Plan, direct and manage all Human Resources (HR) initiatives such as marketing, recruitment, hiring staff, employee relations and termination; Collaborate with senior management to write and implement agency policies; Ensure compliance with New York State Dept. of Health laws and regulations; Maintain staff records; Handle conflicts and complaints received from employees and clients of the agency. Strategize solutions to any issues that arise and followup as needed; Conduct exit interviews to identify reasons for employee termination; Analyze employment-related data and prepare required reports.

• Responsible for comprehensive care management for medically fragile youths 0-21 years old; Created individualized plan of care in accordance with the medical, financial and public/social needs of the client; Arranged for services, supported adherence to medical plans and assisted with school and community services where appropriate; Trained and administered CANS-NY; Identified needs of the child and secured available resources; Maintained excellent notes and records of all case activity.
