- Listens to patient, asks assessment questions and evaluates patient symptoms via telephone or patient messaging to determine severity of patient's condition and recommend the most appropriate course of action.
- Provides medical guidance, assistance and education based on established protocols and provider recommendations in easy to understand terms.
- Communicates with team healthcare members, providers and specialists to ensure continuity of care.
- Accurately documents all patient interactions including symptoms, instructions given, the patient's understanding and agreement or disagreement of provided advisements.
- Reports patient testing, procedure and lab results; including provider's recommendations, orders and referrals.
- Completes medication refills and submits necessary prior authorizations.
- Document, copy, fax and mail completed forms and paperwork.
- Follows all procedures to effectively comply with HIPPA laws and prevent information breaches.
- Participates in mandatory office education.
