- Support and encourage discussions in decision-making processes, within our team as well as practices we work with.
- Identified patterns and trends in large data sets and provided actionable insights.
- Collaborate with practices, workflows and strategies to improve their end of year scorecards.
- Developed customized reports, summarizing and presenting data in visually appealing format.
- Generate standard and custom reports to provide insights into patients in need of certain measures.
- Meet with Payers; United Healthcare, Aetna and Humana to stay informed on any changes and needed focus.
- Meet with our practices to explain their Medicare and Medicaid performance on the Quality Measures.
- Work in different Electronic Medical Records (EMR) to investigate different measures. Such as medication fill history, last A1C, screening histories, diagnosis, etc.
- Work in the payer portals. This is where we can pull gap lists as well as upload findings from the EMR.

- Discuss medication needs with patients and provide assistance with cost, access, alternative medications, education and any other barrier they mention.
- Work with Care Management team discussing solutions for patient medication needs.
- Assisting patients with medication assistance forms and/or online Low Income Subsidy applications.
- Making medication adherence calls, cold calls, to patients that are not showing to be adherent and having discussions on how I can assist them.
- Behavioral Health Pilot - having a small group of patients that have a mental health diagnosis and frequent the Emergency Department. I would have frequent check in calls with these patients, assist with their access to medications, help with organizing transportation needs, helping to organize appointments. I worked along with a License Clinical Social Worker. This work reflected in the decrease in the patients visits to the Emergency Department. (no longer doing this pilot)
