Assist Rehab Manager in supervising and supporting large staff of physical, occupational, and speech therapy field clinicians in home care setting. Responsibilities include: •Coordinate with field clinicians and support staff to ensure timely scheduling and assignment of new referrals •Support and advise field staff on clinical and psychosocial aspects of patient care including treatment strategies, discharge planning.

•Complete chart audits to ensure agency and Medicare compliance

• Instruct newly hired staff on EPIC electronic medical record system, agency policies and practices, coordinate with field clinician preceptors, mentor and provide support and chart audits during provisional hire months.

• Assess need and implement treatment for patients in home care setting with myriad diagnoses COPD, CHF, CVA, post-operative joint replacements, spine surgery, cardiac surgery.
• Assist patient and caregivers in identifying and meeting goals for improved function and mobility in home and community.
• Care management duties including medication education and reconciliation, emergency preparedness education, home safety evaluation and education, care coordination with interdisciplinary team members and physicians.
• Train new hires to field clinician duties as preceptor including scheduling, epic chart reviews, infection prevention in home, agency standards of care, all aspects of medical record charting for agency and Medicare compliance, communication with team members and physicians.
• Re-Hospitalization Work Group member: interdisciplinary team tasked with identifying and mitigating risk of patient re-hospitalization with work place and organizational practices.
