As a Dependency Investigator, Children Social Worker. The following duties were performed;

• Assessment of foster child, and prospective adoptive parents for clearance for licensing and home approval for placement.
• Documentation of foster placement, assess mental health status all participant, request and provide medical, dental, academic, mental health records, & visitation history with birth family to the court.
• Request and review, CLET, CACI, DOJ, & FBI records report, Make referrals to community services, and provide court report containing recommendations regarding the Department's stance of the adoption process to Dependency Court.

As a Family Maintenance Social Worker the following duties were performed;

• facilitate multidisciplinary team meetings quarterly to conduct a 90 day assessments to determine the progress in case plan services and develop a new case plan based on the family's strengths, needs, & adjust goals accordingly
• encourage & motivate to develop new coping skills for recurring problems for individuals and families.
• provide crisis intervention in the form of new referrals based on the client's expressed needs & progress.
• refer family members to community resources & collaborate with community service providers to determine compliance & progress
• provide monetary funds based on family financial need
• assess the house for safety of the child and family
• assess mental health status of all family members and when necessary, refer, collaborate with Department of Mental Health or school psychologist, to obtain progress reports
• meet/collaborate with medical, dental, and educational professionals, and participate in IEP meetings refer for tutoring when appropriate
• upon successful completion of the case plan, terminate the case or promote to court requesting a warrant for removal of the child from the home

FM/FR Social Worker duties consist of the following responsibilities;

• refer family to community resources as needed & monitor progress & compliance progress report
• participate in discharge meeting for youth on 5150 hold to determine safety, placemen, & obtain psychotropic medication information to submit to Court
• visit youth in locked group home facilities
• conduct Risk & Safety Assessments prior to court to determine recommend, and when appropriate, prior to case termination after court has determined that there are no further safety risk.
• collaborate with hospital staff to arrange transportation and obtain medical records for infants/youth due to be released from hospital holds due to safety threats in the home
• conduct in person monthly home/school/placement visits with the child(ren) and collaborate with parents, educators, foster parents, minor's attorney, and relatives as needed to assess ongoing safety, needs, and to facilitate visitation when appropriate
• arrange and facilitate quarterly 90 day assessments Multidisciplinary Team Meetings with community providers to include; DMH, Foster Family Agency, educational staff, parents, & relatives to discuss Court ordered services, the family's strengths and needs, and make adjustments to the case or treatment plan if necessary
• obtain medical, dental, academic, and progress reports from parents/service providers to submit to Court and provide a semi-annual Progress Court Report to Dependency Court with recommendations.
• determine & arrange appropriate placement, and refer prospective caregivers for criminal background clearance

• On the campus, act as a liaison between the Department of Child & Family Services, the school, parents, service provider, and caregivers
• conduct In person monthly assessments of children under court jurisdiction/foster youth students to assess for mental health, safety in home or placement, determine academic needs & progress.
• provide referrals for services
• participate in IEP meetings & collaborate with educational staff, to include school psychologist
• provide safety assessments and recommendations for youth

• Conduct mental health assessments and safety risk assessment and report to multi disciplinary team
• meet with multi disciplinary team to include; nurse, medical assistant, chaplain, & physician for treatment plan review & updates
• Collaborated with multidisciplinary teams to develop comprehensive treatment plans for terminally ill patient & caregiver
• conduct weekly evaluation and provide referrals, inquire as to medication and possible side effects, ensure proper equipment and medication has been provide, and addressed patient & caregiver's needs and concerns.
• provide referrals as necessary and act as a liaison between multi disciplinary team and patient and provide community end of life referral services provider information to family & caregiver
• attend meetings, training, conferences, and seminars to advance professional knowledge.
• Supported clients in crisis situations by providing immediate resources and referrals.
• Provided one-on-one counseling sessions to support client mental health and well-being.
