
•Key member of a multi-disciplinary Community Health Team
•Collaborate with physicians, other care team members (Medical Assistants, Nurses, etc.), hospitals, partner agencies, members of the CHT in the patient’s network of care
•Meet with patients in the community, whether at their home, in the hospital, or another community setting
•Deliver community-based individual, group, couples and or family interventions
•Transport and/or accompany patients to clinical and/or other relevant appointments, as necessary
•Apply knowledge of Psychiatric Diagnosis (DSM-IV-TR & ICD-9) in order to perform and record mental status examination findings and multi-axial diagnostic formulation and billing codes with assigned patient caseload
•Assist in the development, review and delivery of a patient Care Plan to include actions designed to improve the patient’s health status
•Assess clinical emergencies, including risk of suicide and danger to others
•Provide referrals to appropriate resources and follows-up as necessary
•Obtain progress notes from outside providers involved in patient care
•Engage and empower patients to understand their health issues and work closely with patients to take proactive measures that will result in positive outcomes
•Perform timely and accurate record keeping in accordance with all Health Center requirements
•Complete daily activity logs, behavioral health screening & progress notes, encounter forms, reports, evaluations, and insurance documentation as required by pay sources
•Complete clear, specific, and behaviorally measurable treatment plans with assigned patients
•Maintain cooperative relationships with Health Center employees, patients, community agencies and the public
