Completed weekly visits to participant's home to monitor and assess services.

Connected participants to available resources and acted as an advocate for participants.

Scheduled psychiatric appointments for participants and ensured that he/she attended for evaluation and/or medication checks, etc.

Maintained a productivity of one hundred and thirty units per ;week.
