• Develop and implement guidelines and workflows within the ACO with Post-Acute Care Manager for participating skilled nursing home facilities
• Participate in monthly meetings with nursing homes to review data, workflow issues, discuss hospital transfers and re-hospitalizations after discharge, help implement ways to prevent future hospitalizations
