Transitional Care Specialist

- Health care professional whose duties and responsibilities are to support patients and their family as they move through each level of care, mainly form discharge care to home location while helping navigate through different care options at different moments in their recovery or care regimens
- TCS during each home visit checks vital signs, review and go over with patient medication regimen after discharge, ensure correct eating habits with special instructions from discharge, update all primary care physician follow-up appointments and specialty clinic if needed as well as contacting home health, Physical therapy, Occupational therapy and other necessary help needed for each patient
- We help the patient connect with the appropriate program or facility for their level of care, advocate on their behalf, and ensure all parties have the appropriate patient information.
