  • In this role I enroll High Risk patients with prior hospital admissions or ER visits.
  • I conduct a High Risk screener, review their social determinant of health needs, create a care plan to address those needs over 6-12 weeks in the program.
  • I also provide education on their health conditions.
  • We have bi-weekly high risk rounds to present the patients that are currently enrolled in the program.
  • I facilitate and present to Physicians, PAs, NPs, and Associate medical directors as well as all upper leadership attending meetings.
  • I do weekly home visits on patients that are identified to have or need closer following with interventions for the SDOH needs.
  • Refer to local resources HHSC and other agencies to assist patients.
  • We work closely with APS also MHID.
  • I attend meetings as needed.

  • Currently- Scheduling Initial Dietician consultation requests from CM.
  • I was recruited to assist the dietician pilot team to outreach members in getting them scheduled with Dietician consult for the dietician team.
  • I have been successful in getting the Queue down to January of this year and managing the queue with the help of 2 other CMSA’s.
  • We update ICT requests and complete proper documentation in CGX.
