1. Obtains and documents complete medical history and physical data on patients.
2. Interprets and integrates data to determine appropriate treatment plan as needed.
3. Performs a complete physical exam and record findings. Collaborates with physicians in managing acute and long-term medical needs of patients.
4. Identifies normal and abnormal findings in the history and physical exam. Orders appropriate laboratory and imaging studies.

illness prevention. Recommends resources to meet patient and family needs.

15. Collaborates, communicates, and meets with MAT treatment team and other practice care team staff as needed and required.
16. Provide education and assessment to patients looking to access MAT treatment programs.
17. Assess and monitor patients in the induction, stabilization, and maintenance phases of treatment.
18. Works with the pharmacies and the MAT team in obtaining medication orders, prescriptions, refills, prior authorizations, and communication as
needed.
19. Exercises HIPPA confidentiality and security measures always during the office hours and outside of the office.
20. Demonstrates responsibility for self-learning through participation in continuing education activities and conferences.
21. Participates in quality improvement reviews and activities in the clinical setting.
22. Prescribes buprenorphine-naloxone / buprenorphine to patients who are enrolled in the MAT program and who are in compliance with treatment
contract.
23. Trained in Narcan administration and overdose response.
24. Trained in administration of Sublocade, Brixadi, and Vivitrol.
25. Counsel patients on management of their illnesses.
26. Refer clients to psychiatric and other emergency departments, substance abuse and mental health care providers, and other providers, as
appropriate.
