Summary

Since becoming a nurse I have worked with Hospice care in the home setting, home Health Care , Acute Care Hospital nursing for 6 years with a local agency within the Med Surg and Emergency room units. Since 2006 I have been involved in Skilled Nursing in Long Term Acute Care settings. I started my path of SNF as a Assistant Director of Nursing and quickly moved up to Director of Nursing. I have lead trouble teams to successful survey through my dedication to staff development and quality patient care. I am a extreme hands on nurse director who believes in being a Team Leader and development in the trust of staff as well as patient to promote better patient outcomes. I have successfully worked programs of Psychotropic Medication Reduction which we call THE HOPE INITATIVE and also run a UTI program names operation FOXTRAIL which has shown great success in reduction of chronic UTI. Knowledge of Quality Measure and QIPP measures how they impact resident outcomes as well as facility growth. The knowledge and ability to provide great leadership and being a positive role model to gain and retain the support of the facility staff which overall promotes positive atmosphere. As DON and ADON the overall goal has been to provide consistent quality of care to Skilled and Non Skilled patients, development of relationship and trust with physician organize QAPI aswell as monthly standard of care meetings to meet consistent quality of care.
