  • Assisted and supported individuals/families to explore OPWDD eligibility and apply for Care Coordination Organization Home Health (CCO/HH) services utilizing efficient multitasking, organizational, & excellent communication skills in a fast-paced work environment. Coordinated evaluation referrals to Article 16 clinics, confirmed appointments for submission to OPWDD to complete enrollment, Medicaid billing & soft hand-off to Supervision for Care management assignment.
  • Essential Responsibilities included HIPAA regulation while managing a high volume of calls, fielding inquiries from potential enrollees, and referral sources. While assisting enrollees with enrollment, positive relationships were maintained with referral sources and enrollees while responding to requests or concerns. Provided information, education & vitally important customer service to Tri-County Care referral sources or potential enrollees for successful outcomes.
  • Regularly attended outreach meetings and conducted announcements regionally to build organization awareness and filter in more referrals with the Schools of District 75, multi-borough DD councils, and Region 4 DDRO. This increased open enrollment awareness. Maintained a full calendar for multi-borough virtual outreach meetings & informational sessions while maintaining secure capture of Excel data reports for supervision and regional review.
