  • Assess new and existing residents upon admission, annually, and change of condition. Adjusted Service plans as needed.
  • Established familiarity and rapport with residents to effectively gain service feedback and understand residents' needs and preferences.
  • Oversee Care Department consisting of Care-Partners, Medication Department, and Nursing team, 25-30 combined associates.
