Build long-term meaningful relationships by working with the same panel of patients face-to-face in the patient’s home. Work in close partnership with local nephrologists and with the support of a care team, including a social worker and a care coordinator, to lead the effort to help patients have higher quality care, slower progression of kidney disease, and a reduction in traumatic and costly episodes such as avoidable hospitalizations, readmissions, unexpected kidney failure and unplanned or clinically inappropriate dialysis starts. Work with patients to encourage healthy behaviors and ensure patients are well informed and empowered to make proactive decisions about their own care.
Proactively communicates with the patient’s to ensure that if member’s health is deteriorating, make appropriate interventions to get the member seen by the appropriate physician, ensure appropriate medications are prescribed and that preventive, outpatient medicine is delivered wherever possible.
Closely monitor patients to identify signs of arrhythmias and other life-threatening complications.
Responsible for administering prescribed medications and perform treatment of patients as prescribed by physician.
Educate patients and families on disease prevention, healthy diet and exercise, and lifestyle changes.

Assist in resolving billing edits that are holding patient claims from billing, by reviewing medical records and other applicable documentation. Maintain the integrity of facility Charge Description Master (CDM) and Revenue Integrity Systems Program. Maintain the integrity of all Ancillary Department systems related to billing and revenue. Serve as liaison between Administration, the PAS and Ancillary Department Directors regarding revenue and compliance issues. Coordinate all retrospective, concurrent, patient complaint and external billing audits. Provide monthly audit results. Develop and coordinate educational in-services to the Business Office staff and Ancillary staff related to charging and billing issues. Review denial trends for documentation or charging issue opportunities. Review high charge stays/procedures for charging accuracy. Ensures web tool (unbilled) items are addressed properly and timely. Serve as primary contact for all charge-related PAS inquiries and issues.
Work closely with physicians to run a smooth clinic flow. Help facilitate new patient appointments in conjunction with intake team. Record retrieval of patients in EMR. Assist with clinic procedures as needed. Assist with management of patient and staff messaging in EMR. Mailing and calling out benign results. Assist in completion of forms (disability, referral, record release, etc). Sort faxes. Transcribe labs in EMR. Urgent phone calls to assist in patient plan of care. Follow up with patients who did not come to appointments. Educate patients on procedures, health maintenance, and etc. Administer medications as prescribed. Assess patients upon arrival in clinic.
