In this position I was still performing forensic case management duties, but now I was able to begin conducting risk/need assessments using the Level of Service Case Management Inventory (LCSMI), an evidence based actuarial assessment that utilizes the ris-need responsivity model. I also conducted in depth bio-psychosocial assessments in conjunction with the LSCMI. I would initially receive the referral from the individuals defense attorney and I would then make arrangements to meet and asses the individual. Assessments would typically take place at the county jail, the individuals residence, or virtually. I would then use the results of these two assessments to write a comprehensive report and determine if the individual qualifies for a pre-trial diversion program – either Mental Health Pre-Trial Intervention (MHPTI) or Early Offender Diversion (EOD). I then present the results of the assessments and make my recommendations to the court on the record. I would write monthly court reports and testify on clients' treatment progress and program compliance on the record as well.
As a forensic case manager with Gracepoint, I was responsible for all returns from the Florida State Hospital. I meet with them in jail or at their residence, work to find them placement in the community, discharge them from the jail, link them with benefits if they qualify, help them get health insurance, make therapy and psychiatric appointments for them and then sitting in on those appointments, assist them with medication management, help them find employment, and act as a general support system for those reintegrating back into the community. I would also conduct competency restoration sessions with clients who required it, assist with transports to court, and while in court provide testimonies on the client's behalf. I would also draft referrals, sworn affidavits, and conditional release orders on behalf of the clients.
