Major responsibilities included providing advocacy for short and long term patients in the facility. For short term rehab patients, my responsibilities included initial and ongoing assessment of patient's immediate needs and planning for the referral needs, appointments, DME and community resources required for an optimal discharge.
I was also responsible for making sure residents were aware of their rights and aware of the services available to them. This SW was responsible for assessing resident's psychosocial needs and ancillary needs, care planning and making referrals required which included psychiatric services, counseling, vision, dental, audiology and podiatry and or any other referral that could improve and enhance a person's quality of life.
Other duties included admission, quarterly & annual assessments, MDS's, care planning and and care plan conferences.
Presently responsible for 173 residents.
Major responsibilities included assessing rehab patients and identifying resources and support system and discharge needs. Educating patient/family, planning and assisting with discharge needs. This included arranging for durable medical equipment, home health services and connection to community resources.
I was also responsible for addressing long term care residents' psychosocial needs, ancillary needs which included referrals for psychiatric services, vision, dental, podiatric and audiological.
As the social worker I was responsible for maintaining compliance with state and federal regulations.
Responsible for assessing and identifying discharge needs for short term rehab patients. This would include arranging home health & therapy services, f/u appointments with PCP and community support services and resources.
Also responsible for assessing the psychosocial well-being of long term care residents which included assessing cognition and mood to determine what services would benefit them, including referrals for psychiatry, psychology, vision, dental, podiatry and audiology to enhance quality of life.
Basically, the same duties as stated above with the addition of assisting with the Independent and Assisted Living residents as needed. The company closed down the SNF in 2020.
