For the first three years I rotated between pre transplant/in patient transplant and post transplant. During pre transplant I received referrals, requested records and facilitated the patient through the transplant work up. I then presented to our patient selection committee and followed up with the patients I. Regards to the decision. If listing was required, I performed that as well and followed them until they received a transplant.

While on in patient rotation, participated and n daily meeting with APP, social worker and Care coordination, as well as attending daily rounding in the hospital. Rounding was performed in all liver/intestinal patients, hepatobiliary surgery, chronic pancreatitis and carcinoid patients. I provided all post transplant teaching prior to discharge and facilitated out patient appointments.

As a post transplant coordinator I followed multiple labs and testing. Tirades medications, attended all follow up appointments and took patient calls. We follow our transplant patients for life so each post coordinator is following approximately 400 patients.

During the last year I have transitioned to the chronic organ management team where I specialize I. Carcinoid tumors and chronic pancreatitis. I have also been the Rapid City outreach hepatology clinic nurse for the last 3 years. I receive all referral, set up all appointments and fly up to Rapid city 1-2 times a month for clinic

For the first three years I was a pre transplant coordinator where I managed patient evaluations for transplant eligibility, followed all diagnostic studies. Presented at weekly committee meetings for listing in UNOS Evaluate for potential donor organs for transplant suitability and arrange transplant surgery for recipient and shares on call weekend responsibilities as assigned, in order to assure. RN coordination co stage after hours and on weekends.

I performed post transplant duties for two years. I participated in multidisainary team rounds, updated Transplant medical records, prepared patients for discharge and follow up, prepared records for clinic visits, monitored and followed all diagnostic studies, plans, implemented and documented patient and family education, facilitates prescriptions renewals, evaluates patient health status on regular basis, communicates patient issues/concerns to Transplant physicians.
