My current position as HIM Coder I involves the use of advanced knowledge of and skill in applying International Classification of Diseases and Procedures (ICD), and Current Procedural Terminology (CPT), code sets and associated Medicare/Medicaid rules and guidelines as well as commercial insurance payer rules and LCD for commercial payers. I review and interpret patient medical record documentation to identify pertinent diagnoses and procedures and assign ICD-10-CM and CPT codes accurately and timely to the highest level of specificity based upon physician documentation for emergency department, observation encounters, outpatient, ancillary, ambulance, inpatient, long term facility/skilled nursing facility inpatient encounters and clinic visit records. I communicate with physicians and other providers on an -as-needed basis. I maintain current knowledge of coding and documentation changes, rules and guidelines. My daily work includes revenue cycle management charge auditing as well as use of 3M Encoder. I am seeking a position in which I can expand into different medical specialties in order to fully utilize my medical skillset.

• Thirteen years of experience interpreting and typing audio recordings of physicians, surgeons, pathologists, and diagnosticians in 20 medical specialties (multi-specialty clinic; SOAP notes and acute care inpatient History and Physical reports, progress notes, consultations, operative and diagnostic procedures). These transcribed documents became the patients' permanent medical EHR/EMR record.
