Management of time also has a direct impact on colleagues and can be the missing link in a poorly performing team.
  • The Intake team promotes inter-professional synchronization throughout the rehab episode and meets daily to discuss individual plan of care for each patient scheduled for Continued Stay Review.
  • The continued stay nurse must ensure the review is the most pertinent Information to continue the current level of care in a skilled nursing facility and justify the medical necessity of continued hospitalization or care in this facility.
  • Data collection for continued stay reviews are stored in folders on a r-drive. Submission of this data is transmitted electronically via multiple insurance portals or Rightfax. Maintaining the Outlook Calendar ensures timely submission of this data.
  • The criteria for conducting a Review of Continued Stay include:
  • The patient's medical condition and stability.
  • The effectiveness of the current treatment plan.
  • The presence of any complications or new conditions may require a change in the level of care.
  • The patient's ability to safely receive care in a less intensive setting, such as at home or community facility.
