The Intake Coordinator proactively assesses, identifies, and works to mitigate the risks posed by barriers to the patients' transitions of care. This position works collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective care coordination. Management of time also has a direct impact on colleagues and can be the missing link in a poorly performing team.
The Intake team promotes inter-professional synchronization throughout the rehab episode and meets daily to discuss individual plan of care for each patient scheduled for Continued Stay Review.
The continued stay nurse must ensure the review is the most pertinent Information to continue the current level of care in a skilled nursing facility and justify the medical necessity of continued hospitalization or care in this facility.
Data collection for continued stay reviews are stored in folders on a r-drive. Submission of this data is transmitted electronically via multiple insurance portals or Rightfax. Maintaining the Outlook Calendar ensures timely submission of this data.
The criteria for conducting a Review of Continued Stay include:
The patient's medical condition and stability.
The effectiveness of the current treatment plan.
The presence of any complications or new conditions may require a change in the level of care.
The patient's ability to safely receive care in a less intensive setting, such as at home or community facility.
