Per diem position performing all duties in a high performance inpatient pharmacy setting utilizing tech check tech, Epic EHR, KitCheck technology and Pyxis ADM's. Composed accurate and efficient medication reconciliation via patient and family member interviews, SureScripts, CareEverywhere, physician offices, outpatient pharmacies, the VA and Kaiser. Entered iVents to communicate completed medication lists to pharmacists. Answered telephones to respond to questions or requests by nursing, physicians and other disciplines. Performed audits to ensure the 5 rights of medication administration, emailed charge RN's and anesthesiologists, entered PSR's to correct documentation and resolve discrepancies.
