- Coordinating communication between clients and service providers to promote mutual understanding of needs and expectations, with strict adherence to client confidentiality.
- Championing the rights of clients encountering discrimination or obstacles to community services, working to secure equitable access to necessary supports and resources.
- Accurately document all client interactions, interventions, and goal updates to maintain up-to-date records within the case management system.
- Enhancing client participation in the program while connecting them with additional organizations that can help alleviate their challenges.
- Creating a space to support clients with measurable short- and long-term goals. Ensuring that the client (s) program caters to their challenges and is set up to address these concerns and issues.
- Reducing barriers by providing advocacy and intervening in crisis situations when needed
