Six years of experience as a case manager mentor. My job duties included:
Client Assessment-conducting intakes upon a child's admission into the program. Intake process consisted of having the initial contact with clients, their families, gathering personal information, assessing immediate needs, reviewing program's expectations, and completion of intake forms.
Service planning- Implementing and develop case plans based on assessment, ensuring that goals align with client needs.
Documentation: Maintaining accurate case notes, reports, and records keeping compliance with program policies and federal regulations.
Served as a mentor for three years. My responsibilities as a mentor was to be the point of contact (POC) in the absence of the supervisors. My responsibilities as a POC were to run and send daily assessments emails, discharges emails, age out emails, assign intakes, and guide case managers when needed. Additionally, I have provided training presentations for new employee orientation (NEO), created powerpoint presentations about the Office of Refugee Resettlement (ORR) policies.
Staffing- weekly case staffing with case coordinators and Federal Field Specialists (FFS).
Collaboration- Coordinating and collaborated with internal staff such as clinicians, counselors, therapists, and social workers to support the client's well-being. Participated in Treatment Team Meetings when needed.
Crisis Intervention- managed and responded to emergencies as needed. Certified in CPI (non-violent Crisis Prevention Intervention) and CPR.
Family Engagement- Collaborated with client's families at a daily basis. This included interviewing, assessing, conducting background checks, scheduled fingerprint appointments, and worked on Child Abuse and Neglect (CA/N) checks. Provided guidance and helped them understand the case management process.
