Axiom Cares is a hospice home care company, and in this position, I assist with ADLs (bathing, dressing, grooming, and toileting), monitor vitals, document any changes in the patient, help patients with mobility, and provide companionship while working with the hospice care team (nurses, physicians, and social workers).

I provided personal care and assistance to elderly residents in a nursing home setting. This includes assisting with activities of daily living (ADLs) such as grooming, toileting, and showering. I also documented any changes in the resident's condition and worked closely with the other nursing staff.

In this position, I worked on the surgical floor and helped patients with mobility (transferring, repositioning, and walking), I changed external catheters and assisted with elimination needs. I also monitored vitals every 4 hours (blood pressure, temperature, and pulse) and took blood sugars for patients who were diabetic. Documentation was also an important part of this position.

At Alloheim, I helped residents manage their hygiene needs (oral hygiene, bathing, grooming, toileting), assisted with feedings, repositioned residents to prevent bed sores, and provided a safe and clean environment.
