· Through successful completion of general and unit orientation.
· By maintaining unit specific competencies pertaining to the Scope of Care in different stages of life including but not limited to, crisis training, treatment plan development, suicide risk factors, and effects of trauma of survivors.
By attending all mandatory in-services for accreditation (The Joint Commission, ODMH).
Possesses knowledge and skills to assist in the care of patients and families in Behavioral Services. Demonstrates this by:
· Initiates and performs initial screening and assessment including substance abuse assessment as needed - using evidenced based treatment screening tools and guidelines
· Arranges for screening or collaborates with referral sources to arrange for alternate treatment.
· Performs diagnostic and psychosocial assessments, updates and risk assessments.
· Develops, reviews and updates treatment plans.
· Performs counseling/psychotherapy services (including individual, family and in a group setting).
· Provides education and supervision to students
· Provides ongoing crisis intervention for patients and when relevant, their families.
· Acts as an advocate for the patients and externship students
· Initiates and coordinates education regarding treatment for patients and when relevant, their families.
· Provides support for the family as needed post-discharge.
· Participates in Multidisciplinary Planning.
· Identifies legal and custody issues and acts to resolve conflicts that interfere with patient care.
· Arranges for appropriate level of care based on patient need (both in Behavioral Health and Recovery Services)
· Directs and participates in discharge planning activities to facilitate the provision of service across the continuum of care.
Possess skills in communication, both verbal and written.
· Concisely, accurately and legibly records patient information in the medical record and on appropriate documents in accordance with established standards, maintaining patient records of specific clinical services provided.
· Ensures services rendered comply with Medicaid and related regulations
· Documentation is completed in a timely manner and services are appropriately billed for proper reimbursement.
· Communicates inter-departmentally to coordinate patient care
activities. Promotes positive inter- and intra-departmental
relationships.
· Maintains confidentiality and respect in handling all information related to the work environment.
Collects required data to identify patterns and trends and submit reports as requested.
Continuously seeks to improve the quality of Ambulatory Services.
· Evaluates and assists in improving the quality of patient care and
clinical operations in the outpatient setting.
· Actively participates in and supports department quality improvement process initiatives.
· Demonstrates understanding of performance improvement activities.
