· Maintaining sterile fields with application of various dressings

· Tracheostomy care; cleaning and changing humidifier, breathing treatments, suctioning, changing inner cannulas and trach collars

· Trained using various mechanical lifts to transfer patients

· Educating patients and family members for home care and discharge planning

· Manages video/telephone appointments, facilitates group VVC appointments, secure messaging communications to include disease management, whole health coaching, and pre or post intervention follow up calls etc.

· Utilizes teaching materials to promote self-reliance and independence, including methods to prevent complications and facilitate recovery

· Knowledge in VHA health coaching curriculum and patient-centered approaches to care

· Completed the VHA Whole Health Coaching Program offered by the Office of Patient Centered Care and Cultural Transformation (OPCC&CT)

• Educate patients and their families on orthopedic conditions, treatments and prevention
• Monitor and assess patient progress, noting any changes in the patient’s condition
