  • The case manager can complete a priority tool if the individual requests or needs more services
  • This priority tool is reviewed to determine the needs of the individual
  • Priority tools are also completed when someone is referred to the case manager requesting services
  • The case manager's primary concern is ensuring that all individuals on caseloads are safe in their homes and that their rights are always protected
  • The case manager establishes a good working relationship with the vendors contracted out through NWICA
  • The vendors provide case managers with quarterly service progress reports, which are used to monitor the in-home services
  • The case manager is to report any suspicions of abuse or neglect to APS and perform safety checks and phone calls to the vendor if individuals cannot be reached
  • The case manager also completes Pre-admission Screenings and Level IIs as assigned by the PAS Coordinator
  • This is to determine if an individual will remain in the nursing facility or go to a nursing facility following a hospital stay
  • Case managers are to report to their field supervisors
