  • Monthly monitoring anemia and bone management and adjust the medication pre protocol
  • Help patients to solve their problems with MD order or referral to SW and dietitian
  • Communication with provider, social worker and dietitian to address patient's problem
  • Performance improvement by review monthly clinic performance and share the result with team memebers
