Maintained responsibilities of a Medicaid Service Coordinator for a case load of twenty individuals which included nine Class Clients
Had the responsibility of training new employees which included instructing how to properly conduct intake interviews and assessing individual needs; demonstrated the proper procedure for conducting monthly face-to-face contact with individuals as well as quarterly home visits with individuals and family; explained how to properly prepare Service Coordination Observation Reports (SCOR's); taught how to properly plan, prepare and implement Individual Service Plans (ISP's), and ensured they were current and up-to-date and explained to and understood by the individual and family; taught how to properly maintain and update individual's case files, including case notes and the required paperwork required by Office of Mental Retardation and Developmental Disabilities (OMRDD); explained how to gather and maintain documentation for individual's case files and how to serve as a liaison between agencies and individuals; Overlooked other Medicaid Service Coordinators and ensured all case notes and ISP's were written properly and all necessary data were included;
Ensured all case files contained all required paperwork mandated by OMRDD in preparation for audits; Ensured all SCOR's were completed and updated; Collaborated with department supervisor on a monthly basis
