Resident Care Coordinator is the department leader for both the Assisted Living and Reminiscence (Memory Care) neighborhoods, they are responsible for facilitating and leading the resident care teams. Responsibilities include Championing the designated care manager concept throughout the neighborhoods, recruiting, hiring and training a high quality team. Providing recognition and creating an engaged culture for your team members, residents and families. Working directly with the RCD (RN) to create accurate, informative, individualized care plans for each of your residents (Point Click Care). Ongoing care plan meetings to include both residents and their responsible parties. Effectively managing schedule via on-shift to ensure staff labor hours are in line, and budget is followed. Hosting quarterly support groups. Fostering strong and engaged relationships with our professionals in order to ensure the needs of our residents care met in line with their services, and ours.
