• Enroll and provides a level of care coordination to Enhanced Case Management-Like Services program members beyond standard case management for individuals with complex needs
• Serves specific populations of focus, including individuals experiencing homelessness, those at risk for hospital admissions, and individuals with serious mental health or substance use disorders
• Connects members to community supports including non-medical services designed to address social drivers of health, such as housing, transportation, food access, in person and caregiving support and other essential community resources
• Participates in case conferences with interdisciplinary teams to present members’ complex medical and psychosocial barriers and collaborate on support efforts to mitigate these barriers
• Facilitates care coordination discussions across Kaiser specialties, local agencies, and community-based programs, including Community Health Workers and Housing Navigators, to ensure comprehensive and integrated member support

• Provided care coordination for individuals dually eligible for Medicare and Medicaid by connecting members with medical, behavioral, and community resources to support complex health and social needs

• Developed and implemented personalized care plans through comprehensive assessments in collaboration with interdisciplinary teams

• Partnered with local agencies, community based programs, and healthcare providers to ensure continuity of care and expand access to essential services

• Collaborated with multiple agencies including ICWA, cultural brokers, schools, law enforcement, behavioral health, and legal representatives to support family and child welfare

• Conducted comprehensive assessments, substance abuse evaluations, and home visits to develop culturally responsive case plans and ensure client safety and well-being

• Prepared court reports, participated in court hearings, and facilitated Child and Family Team (CFT) meetings and interdisciplinary discussions to guide case planning, service coordination for reunification or permanency

• Provided case management and referrals for housing, employment, transportation, and basic needs while maintaining accurate documentation and ethical standards per NASW guidelines
