  • 1) 200’ standard system with 2”, 3” or 3” pump. Any additional hoses to be charged at $25.00 per section. All HP system billed at $3.00 per linear foot. (Additional labor required)
  • 2) Primer Pack at $25.00 required for all pumps.
  • 3) Helper priced at $75.00/Hour per man.
  • 4) There will be an additional 1.5% Fuel Surcharge on the total invoice of all concrete pumps.
  • 5) There is a 4-hour minimum, and 1-hour travel for all concrete pumps.
  • 6) There is a 4-hour minimum, and 1-hour travel for all 70-Meter and 75-Meter pumps.
  • 7) There will be an additional 20% charge on the total invoice of all concrete pumps for certified payroll jobs, CCIP and OCIP.
  • 8) US American Concrete Finishing has a 4-hour minimum charge based on 8-hour days per man. Overtime beyond 8 hours will be charged at the straight hourly overtime rate.

  • Contractor agrees to the following conditions:
  • A) Accepting a copy of this proposal without signing it and placing a future order constitutes acknowledgement and acceptance of all terms and pricing on the last quotation.
  • B) Customer will dispense area for prime-out and clean-up of pumps. US American Concrete will not be responsible for clean-up as a result of failure to have such area designated or from concrete mixer driver not cleaning up.
  • C) Customer is solely responsible for protecting/cleaning the jobsite from damage as a result of concrete spatter, over spray, and/or blown concrete hoses, etc. US American Concrete is not responsible for protecting/cleaning finished slabs, decks, slabs below the ones being finished or any other exposed surfaces.
  • D) Protection of finished decks and stairs is the responsibility of the contractor.
  • E) Customer is solely responsible for protecting/cleaning the jobsite from damage as a result of concrete spatter, over spray, and/or blown concrete hoses, etc. US American Concrete is not responsible for protecting/cleaning finished decks, slabs below the ones being finished or any other exposed surfaces.
  • F) All finished testing to be done prior to stressing of cables and removal of shoring with VCT, #2 RHS, as mandated by ACL.
  • G) Pour dates to be logged within 24 hours. Any deficiencies are to be reported to US American prior to shoring removal.
  • H) US American not responsible for fitness testing deficiencies on PSI/cambered decks or any non-shored decks.
  • I) Any wet testing to be done prior to stressing of cables and removal of shoring.
  • J) All finishing work for pricing based on 8hr days. Overtime beyond 8 hours to be charged per price list.
  • K) All Division 3.2.0. finishing standards apply for all pours.
  • L) Contractor to provide protection on metal stairs while pouring stairs.
  • M) Contractor to provide template for all spiral stairs. Contractor to properly secure stair nosing for all stair pours.
  • N) Pricing excludes any architectural concrete, salt finish, concrete stamps, and color concrete finishing.
  • O) Approved equal pattern must be submitted in writing to US American prior to commencement of any repairs.
  • P) Beams and/or pattern greater than 2” are considered vertical pours. Vibration in these areas is the responsibility of the contractor.
  • Q) Rebar/electric chair on conduit spacing must be greater than 2” prior to proper vibration and penetration of aggregate.
  • R) Contractor to provide 1/4” walkway and/or personnel for all working decks for proper edge/ depression fitness for access and safety.
  • S) Contractor to supply water, power, and adequate hoses.
  • T) Saturday, Sunday, or Holiday pours will be charged an additional 25% over current rate.
  • U) US American to provide adequate space for the pump truck to the nearest point of placement. US American Concrete will not be responsible for damage to the property line inside the curb line.
  • V) Contractor to provide adequate hoses for all interior pours.
  • W) 24-hour notice of cancellation required for all orders. If timely notice is not given, a 4-hour minimum will be charged, regardless of reason.
  • X) Contractors to have authorized representative on deck throughout the pour. Contractor will also provide a man to back up the mixer truck to the hopper when necessary. Contractor to supply water, hoses, and a man on deck to wheel ahead of pour.
  • Y) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Z) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Aa) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Bb) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Cc) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Dd) Contractor agrees to restore deck and/or ground to original condition. Contractor will not be responsible for damage to the property line inside the curb line.
  • Ee) The contractor agrees to follow all OSHA regulations pertaining to providing a safe work environment. Contractor agrees to pay all municipal and OSHA fines that may be incurred by US American Concrete as a result from the failure of contractor to provide this safe work environment.
