Responsibilities include working alongside an interdisciplinary team of nurses and support staff in order to provide hospice related social work services to patients and patient families across the state of Rhode Island. Hospice caseloads average thirty one clients, with care being provided in both home and long-term care nursing facilities. Services include providing short term counseling support to patients as well as assisting patient families in mitigating and responding to anticipatory grief; as well as connecting families and patients with community resources pertaining to end of life arrangements or additional therapeutic supports as needed. Other responsibilities include facilitating patient care plan meetings, assessing for abuse or neglect, and a continued advocating for patient needs and comfort in both facility and home environments.

Provided individual, group, and family therapy services to client's residing in short-term residential CBAT facility located in New Bedford, MA. Typical client caseload consisted of 4 to 9 individuals, with each client remaining on unit for an average of 8-12 days. Clinician responsibilities also included establishing and maintaining contact with client collateral contacts, creating individualized treatment plans for each client, working alongside a professional team of mixed medical and mental health professionals, and seeking out additional referrals for clients as needed.

Provided in-home therapy services to families located in the Dedham/Boston area of Mass. Caseload typically consists of five to eight families, with each family being comprised on average of three to five clients. Interventions and assessments utilized in treatment were systemic in focus, and occurred in both home and community settings.

Provided community case management services to youth ages 9 to 18 in the South Coast region of Massachusetts. Services included implementing a treatment plan utilizing structured lessons and developing a shared vision plan with the client's family and support network with common goals including socialization and community integration.

Worked alongside Medicaid providers to provide intensive case management services to opioid addicted adults who were considered high risk within the community. Case management encompasses a wide variety of services constructed around client's individual needs, but usually includes assistance with housing, sobriety, income, transportation, and connections to mental health and medical providers.

Constructed therapeutic solutions based on the specific and changing needs of each individual client in order to better create, develop, and implement skills required for successful and productive community-based living.
