Telephonic Nurse Case Manager acts as intermediary to drive appropriate and cost-effective healthcare services for medically rehabilitated individuals by developing patient-centered case management plans. Leverages effective, independent nursing judgment and skills and evidence-based clinical decision-making criteria to optimize and improve health for individuals across care continuum. Demonstrated experience in management of chronic disease process, nursing process and collaborative care planning. Highly motivated professional demonstrates ability to develop and implement successful plans of action to address individual needs. Possesses strong problem-solving skills to reduce barriers. Committed to helping individuals and families. Compassionate Registered Nurse/ CCM with comprehensive knowledge of Workers Compensation programs and community resources. Skilled at identifying needs and developing effective plans of action to provide intervention and assistance. Possesses strong organizational, interpersonal and communication skills.
Direct patient care, second shift, working 3 days weekly on the Oncology Floor. Averaged 6 patients per one RN. Duties besides direct care and assistance with ADL's included hanging and giving blood products, readings and assistance with Radiation Intervention Therapy and administering Chemotherapy.
1) Active unencumbered Registered Nurse license with State of Iowa.
2) Active unencumbered license, Certified Case Manager with the Commission for Case Manager Certification, United States of America.
